Registration

Upon visiting our program, you will receive a packet of information about the center and the classroom(s). An Enrollment Form will be required to hold a spot for your child along with the registration fee of $60.00.

Registration Forms are required on or before the First Day.

Please turn the following forms in to the office:

Immunization record
Enrollment form/card
Placemat/food disk
Emergency care plan
Sunscreen permission slip
Fieldtrip permission slip
Website permission slip
Transportation form for school-age children
Intake form for children under age 2.
Allergy or asthma plans if needed. These forms must have a picture of the child attached and must be signed by the parent and physician.
Special Diet form if needed (this includes milk substitutes) this form must be signed by the parent and physician.
Physical exam forms must be turned in for each child within 30 days of enrollment
