Upon visiting our program, you will receive a packet of information about the center and the classroom(s). An Enrollment Form will be required to hold a spot for your child along with the registration fee of $60.00.
Registration Forms are required on or before the First Day.
Please turn the following forms in to the office:
- Immunization record
- Enrollment form/card
- Placemat/food disk
- Emergency care plan
- Sunscreen permission slip
- Fieldtrip permission slip
- Website permission slip
- Transportation form for school-age children
- Intake form for children under age 2.
- Allergy or asthma plans if needed. These forms must have a picture of the child attached and must be signed by the parent and physician.
- Special Diet form if needed (this includes milk substitutes) this form must be signed by the parent and physician.
- Physical exam forms must be turned in for each child within 30 days of enrollment
